Parents are required to make sure their scout’s medical form is up to date. We suggest you file one copy with the troop and keep another copy that your scout can bring with them to camp outs and summer camps. If your scout does not have an up-to-date medical form, they will not be able to camp. In order to avoid confusion, pick a date such as Sept. 1 (start of school year) or Jan. 1 to renew your son’s physical.

Also, all Adults and Leaders who will attend troop events and outings must have a medical form on file with the Troop.

Here is the required form. Please be sure to complete Sections A, B, and C. and submit to the troop's Medical Records Coordinator.
